Final Fantasy III uses a table of 256 values that it “randomly” loads from, which then returns the psuedo-random-but-predictable-number. What this hack does is eliminate the old random number generator, of course. It is replaced with something that the hacker at least hopes is better in the long run.
The old one was a lookup table of 255 values which were “randomly” loaded. The new one adapts a constantly-tweaked frame counter in with the lookup table. Hence, the name “Holy Randomness Batman!” has more meaning to it than just being a simple joke title. Feel free to download this hack and see how the new number generator impacts the game.

Answer : Watch this video for instructions on what to do. When you download this hack the download will contain a .ips file (Not a ROM file), This .ips file is a patch which you need to apply to the original ROM in order to be able to play the hacked game. The easiest thing to do is copy the .ips file in to the same folder as the original ROM and make sure the .ips and .rom file have the same name, for example

Now launch your emulator and then open the ROM and you should be able to play the hack. If the hack does not load you will need to patch the ROM file with the IPS file.
